The pilot programme for the Diabetes Physical Activity Referral Pathway is now ready to commence and requires the use of appropriate community facilities in areas with a high prevalence of Diabetes.
There will be four pilot programmes located within North, South, East and West of the city. Active Belfast would like to invite interested parties to complete an Expression of Interest Form Diabetes Referral Pilot for use of community facilities and Active Belfast will provide a specialist coach to deliver the programme.
The Closing Date for all Expression of Interest Forms is Monday 18th July 2016.
